The Community Health Work Coalition of Washtenaw and Livingston County (CHWC) is a collaborativestakeholder group in Washtenaw and Livingston Counties. This group comprises community healthworkers (CHWs), CHW supervisors, and representatives from a variety of local entities including hospitalsystems, federally qualified health centers, county government, nonprofit organizations, and researchinstitutions. CHWC is organized by the Michigan Community Health Worker Alliance (MiCHWA) andTrinity Health. While Washtenaw and Livingston Counties often rank near the top of Michigan’s healthrankings and other social determinants of health (e.g., average income), there are substantial disparitiesin access to care, health outcomes, and social vulnerability across the counties.
